6/8/2023 EnchantaVaughn Approved Behavior: Anthony did not communicate his feelings at the beginning of session and remained quiet while QMHPC asked about what was on his mind. Once he was able to process his emotions, Anthony described that he worries about issues between his parents, among other things. Anthony communicated that he didnât feel like talking about his day. Behavior: Anthony was given praise and reinforced for participating in the session and developing self-determination skills. Anthony continues to display appropriate social skills in therapeutic setting. Anthony demonstrated ability to multi task by working independently and helping his mom when she needed. Anthony showed he is capable of exhibiting responsible behavior at home by being fair towards his sister and brother who wanted to play with some of his things. Although Anthony had a hard time paying attention and sitting still he was careful not to talk over QMHPC while she was going over the instructions to the therapeutic activity. Intervention: QMHPC educated Anthony on behavior modification to help Anthony identify what behavior he needs to decrease and avoid daily. QMHPC utilized behavior modification system to aid the Anthony with following directions given by authority figures. QMHPC explored the Anthonyâs capability to cope with his stressors and to get back on track. QMHPC modeled the correct behaviors and conduct during quiet time. QMHPC assisted Anthony with finding and utilizing necessary resources. QMHPC verbalized the awareness that all people are unique, learn differently, and have various strengths and weaknesses that can either hold them back or push them forward. Intervention: QMHPC utilized cognitive skills to give the Anthony a chance to openly and honestly discuss his feelings about his current state. QMHPC utilized active listening skills and was empathetic with the Anthony as he vented the overwhelming events that had occurred, resulting in him taking his frustration out on his mother. QMHPC addressed the Anthonyâs thinking errors. QMHPC encouraged Anthony to be respectful to authority and peers. QMHPC reminded Anthony to build healthy relationships with his family and use them for support.. QMHPC reiterated the importance of positive thinking and exercising his coping skills. QMHPC will assist with reducing anger-provoking situations through self-talk and role play scenarios. Response: Anthony discussed mental illness and its causes as a family. Anthony displayed interest in learning about Attention Deficit Hyperactive Disorder. Anthony believes he will get better if he continues to work hard. Anthony was acceptive of support and encouragement. Anthony exhibited good social behaviors. Anthony was able to reevaluate himself and display a more positive attitude toward peers and authority. Anthony has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Anthony demonstrated expectations and reported a decreased stress level due to his modulation of social contact. Response: Anthony responded well to feedback, interacting well with others. Anthony implemented his plan for restoring relationships, and this was reviewed. Anthony acknowledged QMHPC and explored his options. Anthony proceeded to reduce the aggression in his voice while calming down. Anthony accepted suggestions from QMHPC as evidenced by the Anthony smiling and saying thank you.
